Agglutination occurs when antibodies attach to antigens on the membranes of neighboring red blood cells, linking the red blood cells together. The most common cause is the "cold IgM antibody that causes agglutination", a phenomenon that does not cause noticeable symptoms in the patient, who has a body temperature of 37°C, but which occurs after exsanguination; sometimes the patient may present with Raynaud's phenomenon. In some cases, however, its effects may have clinical significance, since the antibodies may activate complement leading to hemolysis, or agglutinated cells may cause occlusion of small blood vessels in the cold (acute erythrocytosis). Also agglutinated cells can accelerate sedimentation, leading to an elevated erythrocyte sedimentation rate (ESR).
